Built for Medicare-certified CORFs

Documentation, billing, and survey readiness for a CORF, in one system.

CORFs operate under requirements no other outpatient setting carries. Physician-signed plans of treatment. Three core services delivered in a coordinated rehabilitation program. Recertification by a facility physician every 90 days. Survey readiness with documented emergency preparedness, patient assessments, and interdisciplinary care coordination. StrataEMR is built to keep claims clean, documentation defensible, and surveys uneventful.

How it works

Built for the CORF setting from certification to claim

Four places StrataEMR enforces CORF-specific rules so documentation holds up under survey and claims pay clean.

Step 1

Physician certification, captured at the patient level

CORF rules require the certifying physician to establish and sign the plan of treatment before the start of treatment, and to recertify the plan every 90 days for PT, OT, and SLP services or every 60 days for respiratory therapy. NPP-signed certifications are not accepted in the CORF setting. StrataEMR enforces physician-only certification, captures the certifying physician's NPI, tracks the certification date, and surfaces upcoming recertifications as tasks before the window closes.

  • Physician-only certification enforced at the document level
  • Certification date and NPI captured per patient
  • Recertification windows tracked as tasks (90-day and 60-day)

Step 2

Coordinated rehabilitation program in a single chart

The three core CORF services (physician services, physical therapy, and social or psychological services) and any optional services (occupational therapy, speech-language pathology, respiratory therapy, nursing, prosthetic and orthotic devices) share a single patient record. Discipline-scoped templates ensure clinicians document within their license.

  • All disciplines share one interdisciplinary patient chart
  • Templates scoped to each clinician's licensed discipline
  • Physician, PT, OT, SLP, RT, and social work documented side by side

Step 3

Survey-ready documentation and access controls

CORF surveys verify the three core services are delivered, that the rehabilitation program is coordinated and skilled, and that the facility maintains an emergency preparedness program reviewed at least every two years. StrataEMR's amendment history with versioning, required selections per template, Staff Type Required, document completion blocks, and Two-Factor Authentication produce documentation that holds up under survey.

Step 4

CORF-aware claims operations

CORF claims have specific revenue code, place-of-service, and service-furnishing rules. Therapy services are generally furnished on the premises with limited exceptions. CORF physician services are administrative, not separately billable as physician services. The StrataPT billing team works inside the EMR under those rules and runs claims, denial follow-up, appeals, and ERA posting without a secondary platform or external handoff.

  • CORF revenue codes and POS 62 enforced on every claim
  • Physician services kept administrative, not separately billed
  • Denial follow-up, appeals, and ERA posting by the StrataPT team

What's in the workflow

Every CORF requirement, covered

Physician-only certification, interdisciplinary documentation, survey-ready amendments, and CORF-aware billing in one platform.

Physician-only certification enforcement

Plans of treatment cannot be completed without a facility physician's NPI on file. NPP-signed certifications are blocked at the document level in the CORF setting.

90-day and 60-day recertification tracking

PT, OT, and SLP services tracked on a 90-day recertification window. Respiratory therapy on a 60-day window. Upcoming recerts surface as tasks before they close.

Interdisciplinary patient chart

Physician, PT, OT, SLP, RT, and social or psychological services share one patient record. Coordinated care visible to every discipline at the point of care.

Plan of Treatment template

CORF-specific Plan of Treatment template requires diagnosis, goals, treatment minutes, and the certifying physician's NPI before completion.

Required selections per template

Each template carries required fields marked in red until filled. Discharge plan, treatment minutes, and certifying physician all block completion when missing.

Staff Type by discipline

Clinicians see only the templates and cases for their licensed scope. PT, OT, SLP, RT, social work, and physician roles each carry their own Staff Type.

Document amendment history

Every edit after completion creates a new version with reason, clinician, and timestamp. V1, V2, V3 visible to surveyors without leaving the chart.

Document completion gating

Notes cannot be marked complete without an active policy on the date of service, completed benefits, certifying physician NPI, and all required selections.

Direct Medicare connection

EDI connection to Medicare for eligibility and claims. Benefits auto-verify on policy save. CORF rules applied at the claim level for revenue code and POS.

StrataPT billing team that knows the CORF rules

In-house billing staff run claims, denial follow-up, appeals, and ERA posting inside the EMR with CORF-specific rules applied.
