How it works
Four places StrataEMR enforces CORF-specific rules so documentation holds up under survey and claims pay clean.
Step 1
CORF rules require the certifying physician to establish and sign the plan of treatment before the start of treatment, and to recertify the plan every 90 days for PT, OT, and SLP services or every 60 days for respiratory therapy. NPP-signed certifications are not accepted in the CORF setting. StrataEMR enforces physician-only certification, captures the certifying physician's NPI, tracks the certification date, and surfaces upcoming recertifications as tasks before the window closes.
Step 2
The three core CORF services (physician services, physical therapy, and social or psychological services) and any optional services (occupational therapy, speech-language pathology, respiratory therapy, nursing, prosthetic and orthotic devices) share a single patient record. Discipline-scoped templates ensure clinicians document within their license.
Step 3
CORF surveys verify the three core services are delivered, that the rehabilitation program is coordinated and skilled, and that the facility maintains an emergency preparedness program reviewed at least every two years. StrataEMR's amendment history with versioning, required selections per template, Staff Type Required, document completion blocks, and Two-Factor Authentication produce documentation that holds up under survey.
Step 4
CORF claims have specific revenue code, place-of-service, and service-furnishing rules. Therapy services are generally furnished on the premises with limited exceptions. CORF physician services are administrative, not separately billable as physician services. The StrataPT billing team works inside the EMR under those rules and runs claims, denial follow-up, appeals, and ERA posting without a secondary platform or external handoff.
