Built for Therapists

Less time charting. Fewer denials. Cleaner claims.

StrataEMR pre-populates documentation, captures sessions with Mako AI Scribe, and catches missing authorizations before they create issues so notes get done before your day is over.

How it works

A documentation workflow that finishes itself

Four moments where StrataEMR shaves time off the chart and pulls compliance work upstream of the claim.

Step 1

Start the note from the schedule, not from scratch

Click the clipboard icon on a scheduled appointment and StrataEMR routes you based on the appointment type. Evals, progress reports, and discharges open the inheritance picker. Routine treatments auto-clone the most recent note to the current date of service.

  • Single click from schedule to open document
  • Auto-clone for routine daily notes
  • Smart routing for evals, re-evals, and discharges

Step 2

Document with Mako AI Scribe in the room

Mako AI Scribe records the session, generates a SOAP-organized summary, and produces a full speaker-labeled transcript. With Mako fragments in your template, one click transfers Subjective, Objective, Assessment, and Plan content into the corresponding sections of your note.

  • SOAP-organized summary generated from the session recording
  • Full speaker-labeled transcript available for reference
  • One-click transfer pushes content into Mako fragment sections

Step 3

Inherit instead of retype

Three creation paths for every document: Inherit Previous, Inherit Patient Intake, and Start From Blank. Goals, diagnoses, medical history, and Plan of Care details carry forward at the section level. New patient with intake on file? Demographics, history, medications, and allergies pre-populate from what the front desk already collected.

  • Section-level inheritance preserves customizations
  • Intake content flows into Initial Evals
  • Clone any completed document for repeat re-evals

New Document · Re-Evaluation

Inherit Previous
Carries forward goals, diagnosis, and Plan of Care from the most recent completed document.
Inherit Patient Intake
Pulls demographics, medical history, medications, and allergies from intake forms.
Start From Blank
Use the template as designed, with no pre-populated content.

Step 4

Catch issues before the claim goes out

StrataEMR will not let you mark a document complete if the visit is missing an active policy, has a pending benefit verification task, lacks a required authorization, or is missing a referring provider with NPI on a Medicare patient. Documentation errors that cause denials get flagged at completion, not after submission.

  • Active policy required for the date of service
  • Authorization tracked against visit limit on file
  • Referring provider with NPI required for Medicare

"Before StrataPT, every aspect of our billing was done manually — Now, everything is handled on autopilot. I don't have to submit bills or follow up on claim denials, but can still trust it's getting done right."

Built for the way clinicians actually work

Everything in the room with you

Documentation, scheduling, AI assistance, and compliance controls in one workflow.

Documentation built in three layers

Templates, sections, and fragments structured so prior content carries forward correctly across an episode of care.

Mako AI Scribe

SOAP summary plus full speaker-labeled transcript. Transfer to Documentation pushes content into Mako fragments with one click.

Mobile-friendly schedule

Today's agenda, patient names and addresses, call and text quick actions, and one-tap document start.

Map view for home health

Day's appointments plotted with drive times, distances, and route suggestions when a more efficient order is available.

Document inheritance

Inherit Previous, Inherit Patient Intake, or Start From Blank. Section-level inheritance keeps customizations consistent.

Amendments with version history

Once a document is billed, edits go through an amendment workflow with the correction reason captured. No silent edits.

Review notes and document routing

Send a document for review or correction without sending it back to draft. Standard workflow for PTA and COTA sign-off.

Outcome measures and pain drawings

Standardized assessments and visual pain drawings included alongside clinical templates from the Clinical Documents page.
